EHS Athletics
The Ephrata School District uses FinalForms, an online forms and data management service for all EMS and EHS athletics, activities and clubs registration. Follow these steps to begin the registration process.
Things to do BEFORE attending first practice:
- Complete EHS Athletic Registration
- Current Physical Submitted to EHS Office or uploaded in FinalForms (Physicals good for 24-months)
- Pay ASB Fee card ($30) and spirit pack items at EHS Office or complete paperwork if you qualify for fee waiver for ASB Fee
- All school fees and fines must be paid prior to participation.
